The Christmas assistance program registration is open from October 1st to October 31.  To qualify, you must live in the MISD area and have children 18 and under.  Follow the steps below to apply.

1 - Apply online at https://form.jotform.com/242533618559161  or scan the QR code below.

2 - Come to one of the registration locations listed below

3 - Bring the following required documentation: For each child have one school record for school aged children & Birth certificate for children under 5 year.  Adults will need to have both a personal identification with photo card and proof of address.

After you complete your online application, bring documentation to one of the following locations:

October 5th, 9am-12pm: Walnut Ridge West, 7550 Rendon Bloodworth Rd

October 12th, 9am-12pm: Bethlehem Baptist Church, 1188 West Broad St.

October 12th, 10am-1pm: First Baptist Mansfield, 1800 East Broad St.

October 16th, 1pm-6pm: First Methodist Mansfield, 777 North Walnut Creek Dr. 

October 19th, 9am-2pm: St. Jude Catholic Church, 500 East Dallas Street 

October 23rd, 6pm-8pm: Creekwood Church, 7701 Rendon Bloodworth Rd 

October 30th, 6pm-8pm: Mansfield Mission Center Clinic, 901 West Broad St.

October 31st, 10am-1pm: Mansfield Mission Center Thrift Store, 703 East Broad St.
